You MUST have ALL of the following or your registration will be rejected.
- A legible copy of the players birth certificate.
- A Sports Physical signed within the last year (cannot be older than June 16th 2024).
COACHES: Wondering if you have all you need to register yourself?
You MUST have the following or your registration will be rejected.
A screenshot of your current background check completed through the city of Moore

A screenshot of your proof of pending background check completed through the city of Moore.
